Parental Consent

Information required for deed polls for minors under 16

The number of parents/guardians who must sign depends on how many people have parental responsibility (e.g., if 2 people have parental responsibility, both must sign). For children under 16, the deed poll must be executed by all people with parental responsibility. For 16-17 year olds, the child can also sign the deed poll alongside a parent/guardian.

Important Legal Requirement

Parent(s)/Guardian(s) must physically sign the deed poll document at the address(es) provided below. This is a legal requirement for the validity of the child's deed poll under UK law.

The address(es) you enter will appear on the deed poll document as the location(s) where it was signed and executed.

Would you like the Complete Name Change package?

Without the Complete Package you won’t receive step-by-step guidance, template letters, or a tracking dashboard. Most customers find updating organisations on their own time-consuming and confusing.

Government departments including HM Passport Office, DVLA, HMRC and the NHS do not accept photocopies of a deed poll. Banks and other institutions also require an original.
