Your deed poll must be signed in front of a witness — choose how to provide their details
A deed poll is only legally valid when signed in the presence of an independent witness (aged 18 or over, and not a family member). You can either enter your witness details now, or leave them blank and fill them in by hand when you sign.
Most people choose 1 witness. If you don’t have their details yet, no problem — the fields will be left blank on your printed deed poll for you to complete by hand.

Most organisations keep your deed poll on file and will not return it, so you will likely need several copies.
Please indicate the number of additional legal copies you would like to order. We recommend requesting at least 8 extra copies, as photocopies are generally not accepted.
You will need to send your original Deed Poll to all relevant record holders so they can update their records. In most cases, an original Deed Poll is required for the following:
Government departments including HM Passport Office, DVLA, HMRC and the NHS do not accept photocopies of a deed poll. Banks and other institutions also require an original. Some organisations may not return your deed poll once submitted. Please make sure you order enough certified copies to cover all of the organisations you need to notify.
