You need two independent witnesses — people who know you personally but aren't related to you or living at your address. Most people ask friends, colleagues or neighbours. One witness is enough for the deed to be valid in law, but banks and HM Passport Office set their own rules and a number of them ask for two, so your deed poll prints space for both.

Not a copy — an unenrolled deed poll has no central register, so no official copy exists, and a photocopy or scan is not accepted as an original. What you need is a replacement deed poll: a new deed poll confirming the same change, which HM Passport Office, the DVLA and every UK bank accept exactly as they accepted the original.

Both are posted free by Royal Mail Tracked 24, the same as a deed poll. Both statutory declarations are then sworn locally, in front of a solicitor or commissioner for oaths, for the £5 fee fixed by law.

A deed poll you sign today records a change made today, so on its own it cannot evidence a change you made years ago. HM Passport Office accepts an unenrolled deed only where it is a contemporaneous document — one made at the time the change happened — and its guidance asks for a statutory declaration where you cannot show contemporaneous evidence of every link between your old and new names, including multiple changes.

It is not a name change: nothing is renounced, nothing is abandoned and no new name is adopted — it explains the versions that already exist.

The fee is fixed by law, not by the firm:

• Your documents are described in the declaration rather than produced with it, so no further statutory fee arises
• Some firms add an administration charge of their own on top, so budget from £5
